TELEHEALTH POLICIES: 

• Patient agrees to be in a quiet private room during all virtual encounters for HIPAA and privacy reasons. 

• Patient agrees to never log in or answer virtual appointments while driving.

• Patient MUST be in the state of TENNESSEE to complete virtual visits due to licensing restrictions, as Amber Kerby, APRN is ONLY licensed in the state of TENNESSEE and cannot legally practice medicine outside of the state of TN. Patient agrees should they be out of state during scheduled appointment, they will reschedule no later than 24 hours prior to the appointment. Failure to do so will result in the cost of the appointment being charged in full day of, and patient will be asked to reschedule should they log in while in another state that is not TN. 

• Patient agrees to be of presentable dress during virtual visits as if they were in the office. If patient logs in while (and not limited to) topless/shirtless, using the bathroom, in a bikini, or using substances including THC or alcohol during the appointment, the patient will be asked to reschedule and charged the full cost of the appointment.
